The plan pays $250/day (days 1–90) and $600/day thereafter for hospital treatment of any of these specified dread diseases:
Who can be covered
Individual (Plan 5KS): Ages 0–69 · Single Parent (Plan 5KU): Ages 15–69 · Family (Plan 5KT): Ages 15–69
Family rates are based on the age of the oldest covered member. Children are covered to age 21 (or 25 if full-time student). Policy is guaranteed renewable for life — it cannot be cancelled as long as premiums are paid on time. Monthly payment must be by bank draft.
30-day waiting period
Coverage for cancer diagnosed during the first 30 days only applies to expenses incurred after 2 years from the policy effective date. No First Occurrence Benefit is payable for cancer manifested during the waiting period.
- Applicants with a personal history of cancer, leukemia, Hodgkin's disease, or melanoma are not eligible
- Precancerous conditions are not considered cancer under this policy
- Treatment for any disease other than cancer or a specified dread disease
- Treatment outside the continental United States (except emergency basis)
- Treatments not approved by the American Medical Association
- Drugs not approved by the FDA for cancer treatment
- Services for which no charge is normally made in the absence of insurance
- Chemotherapy drugs cannot be claimed under both the Radiation/Chemo benefit and the Prescription Chemo Drug benefit simultaneously
